Required Medical Documentation

For the 2018-19 School Year:

For new students entering the District during the school year, a current physical with up-to-date immunizations is required before starting.

Entering Kindergarten:

Physical / Immunization Proof that the student received a health examination, with proof of the immunizations against, and screenings for, preventable communicable diseases, as required by the Illinois Department of Public Health within one year prior to entering kindergarten.
Vision Exam  Due October 15, 2018 
Dental Exam  Due May 15, 2019

 

Entering 2nd Grade:
Dental Exam  Due May 15, 2019

 

Entering 6th Grade:
Physical / Immunization Proof that the student received a health examination, with proof of the immunizations against, and screenings for, preventable communicable diseases, as required by the Illinois Department of Public Health within one year prior to entering 6th grade.
Dental Exam  Due May 15, 2019

 

Students Transferring
From Out Of State:

Physical / Immunization Students transferring from out-of-state after October 1 must submit a health examination on an Illinois physical form at the time of registration. Students will be given 30 days to submit proof of required vaccinations.
Vision Exam  Due October 15, 2018, or upon entry. 
Dental Exam  Due May 15, 2019, or upon entry.

    The District Wellness Committee developed guidelines for foods available to our students during the school day. Student snacks for birthday and classroom celebrations need to be selected from the current allergen free snack list. In addition, non-edible suggestions are listed.
